Fortaleza provides additional support for students who need more than universal campus services. Services may include individual and small-group support, Tier 2 and Tier 3 interventions, trauma-informed student support, attendance and student reengagement efforts, and mental health referral and resource coordination. Our goal is to expand campus capacity while working within existing systems so students receive coordinated, meaningful support without creating another disconnected layer of services.

Graduation is an important milestone, but what happens next matters just as much. Fortaleza helps students turn career exploration, CTE training, certifications, and graduation plans into meaningful next steps through career and postsecondary planning, résumé and interview preparation, workplace readiness, job development and employer connections, college and technical school navigation, and work-based learning and employment support. We help students understand not only what opportunities are available, but how to access them and successfully move into the next stage of their education or career.

Transition planning should prepare students with disabilities for life beyond graduation. Fortaleza brings experience across disability services, mental health, career development, and employment support to help students build realistic plans for adulthood. Services may include individualized transition and employment planning, career exploration and vocational assessment, self-advocacy and accommodation preparation, TWC Vocational Rehabilitation coordination, and navigation of employment, college, training, and community resources. We help students build the skills, confidence, connections, and support needed to successfully transition into adulthood.

Strong student outcomes are easier to achieve when families understand their options and feel connected to the school community. Fortaleza’s bilingual English and Spanish team supports schools through family outreach and education, parent workshops and resource navigation, attendance and reengagement initiatives, community partnership development, and family resource nights and school events. We help schools move beyond simply sending information home by creating meaningful opportunities for families to participate, ask questions, access resources, and stay connected to the support available to them.

Every school and community is different. Fortaleza services can be customized around district priorities, available funding, student populations, and existing campus resources. Support may be provided during the school day, after school, in the evening, during summer programming, or through targeted short-term initiatives. We can support individual campuses, district-wide initiatives, grant-funded programs, and community partnerships.

Fortaleza does not come in to replace your counselors, educators, or student support teams. We work alongside them.

Our role is to help fill capacity gaps, connect systems, expand specialized support, and help schools move initiatives from planning into action.
